Price by dose
Zepbound is a weekly shot. Without insurance, the main way to buy it is as single-dose vials from LillyDirect, Eli Lilly's online pharmacy. Prices are for 28 days.

On 7.5 mg and higher, refilling late can raise your price by up to $250 that month. Set a reminder a week before you run out.
What else you need
Vials don't come with syringes or needles. You buy those separately, so add a few dollars to your monthly cost.
If you have Medicare
Through the Medicare GLP-1 Bridge, eligible Part D members pay $50 a month for the Zepbound KwikPen. Vials and single-dose pens are not covered by the Bridge. See who qualifies.
If you have commercial insurance
- Your plan covers Zepbound: the Zepbound savings card can bring a fill down to as little as $25, with savings up to $100 a month and $1,300 a year.
- Your plan doesn't cover the KwikPen: the savings card lets you buy it for $299 (2.5 mg), $399 (5 mg) or $449 (7.5 mg and up), with the same 45-day refill rule. Up to 11 fills a year.
Both savings card offers end December 31, 2026. They can't be used with Medicare, Medicaid or other government plans.
